Responsibilities include, but not limited to:  

• Provide psychotherapy to individuals, minors & families 
• Minimum caseload of 15 clients per week 
• Provide progress notes for each session, and Treatment plan every 3 months 
• Keep up to date with Electronic Medical Records 
• Provide phone consultation when deemed necessary 
• Collaboration with outside professionals for the best interest of the client 
• Uphold Ethical Practice Standards and HIPAA Compliance 
• Incorporate a holistic treatment plan rooted in Fundamental Health Solutions Philosophy 
• Independently and efficiently manage schedule in accordance with company policies 
• Provide resources/referrals when appropriate 
• Building of Networking relationships and Referral Sources 
• 1 Blog submissions per month for Fundamental Health Soltuions Blog 
• Attendance to Team Meetings 
• Reliable access to WiFi and privacy for Telehealth 
• Willingness to work in a hybrid Telehealth/In-Person model 
• Be respectful, professional, and ethical
   

Requirements:  

• Minimum caseload of 15 clients 
• Good standing Licensed: MSW, or LMSW 
• Current Individual Practice Liability Insurance Coverage 
• Insurance paneled preferred, but not a requirement* 
• Preferred Certifications/ Experience: BSP, EMDR, EFT, Play Therapy, Art Therapy, Integrative Medicine, Nutrition & Mental Health, Mindfulness, CBT, Trauma, Meditation
